I hereby authorize Premier Property Management to initiate credit entries to my Checking and or Savings account indicated above at the depository financial institution named above and to credit the same to such account. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. 

This authorization is to remain in full force and effect until Premier Property Management has received written notification from me of its termination in such time and manner as to afford Premier Property Management and [Bank] a reasonable opportunity to act on it.
